Guides to Health Care Changes

Integrated Care Program Toolkit:
The Integrated Care Program requies that all Medicaid recipients who are 19 years old or older and reside in suburban Cook, DuPage, Kankakee, Kane, Lake, and Will counties enroll in the program. If you or your child could be affected by this program, please see the Arc of Illinois toolkit.
The goal of this toolkit is to aid consumers and families in their decision-making process regarding these recent changes to Medicaid health insurance. Hopefully, it will make enrolling in the Illinois Department of Healthcare and Family Services' new Integrated Care Program clearer and simpler.
Download the toolkit here.
Children's Memorial List of Service Agencies for Children and Adults with Spina Bifida:
Children's Memorial Hospital has published a list of service agencies that can help individuals with spina bifida. The agencies are on a timeline to show which agencies may be the most helpful to individuals in specific age groups.
This document is particularly helpful now as the Medicaid program is changing in the Chicagoland area.

Children's Memorial Transitions Guidebooks:
Children's Memorial Hospital's Spina Bifida Clinic has published a set of guidebooks to help assist teens and young adults with Spina Bifida.
The first guidebook is aimed to help teens with their transition to adult healthcare, both personally and institutionally.
The second guidebook focuses on lifestyle and goal setting. Its objective is to help teens and young adults become independent while simultaneously planning for their futures.
